I have been wondering how this could have been with the implementation, and the regular reviews, of all these many processes and procedures that ensure accuracy, fraud prevention, and improvements in the efficiency of public financial and operational management, especially, payroll accounting. These include making sure that the processes and procedures do not come under the control of any one person.

This unfortunate situation is, however, an indication of gross incompetence on the part of the Auditors, which must not be taken lightly. They have all along been too much fixated on fraud detection and the persons involved in their approach to work, as if that is the sole purpose of auditing. To the neglect, mostly, of the other aspects of verifying and authenticating the implemented processes and procedures (internal controls) that ensure accuracy, prevention of the very fraud they are much fixated on and improvements in efficiency. That is one of the reasons why we keep reading/hearing about fraud, year in, year out from their reports, with no solution in sight. And the worrying part of this situation is that they are, in most instances, unable to secure any conviction in the court of law for the reported fraud. They turn into a back and forth battle between the two A-Gs and the courts.

But the error, being described in this reportage, is that of an “error of commission”. That is, an amount unintentionally posted to a wrong account. In this case ¢427, 995,661.40 was wrongly described as posted to Ministry of Defence (Wages and Salaries Account), instead of the Ministry of Education (Wages and Salaries Account).
