"Miscommunication" is one of the biggest cop outs in our society. "oh, it was miscommunication". People need to learn individual responsibility which leads to proper accountability. Sad case for sure.

Few months ago, this happened to a gentleman I know. He was married with two children; one was just a baby of two months of so.

How can this be happening in this modern time. I understand that in UK, the one who works on fault line is the only who puts back the power when he/ she has finished his job.

No one else would have the key to put back power on the line except the worker only.

ECG must be fined dearly for this death and the family of the deceased must be compensated heavily.

And some officials must be sacked.

Ghana Government Needs to act. It is not proper to say goodbye to your family saying you are going to work and the next time they hear from you, you are dead.
NOBODY WANTS THAT TO HAPPEN TO HIS OR HER FAMILY.
In most countries before you start work it is a requirement that you carry out Risk Assessment:
In this you work out what could go wrong and take corrective measures to address them and document them. And all workers would be briefed about these before work starts.
For electrical work, all isolated points are locked out and every worker on the job will fit his own personal locks at that point. In other words the power through the isolated point cannot be turned back on until all the padlocks have been removed. The padlocks are clearly identified and no two keys are the same.
Those of you who worked at safety critical plants like VALCO will have heard about personal protection.
Ghana should have Health and Safety Inspectors who will ensure that safety issues are identified in our work procedures and correctives measures are put in place to address them.
The procedures apply to emergency situations well

Ghana Government Needs to act. What we need is an ACT of parliament to address issues like these and employers are held accountable for the health and safety of all their employees. Employees will also have responsibilities under the ACT.

It is not proper to say goodbye to your family saying you are going to work and the next time they hear from you, you are dead.

NOBODY WANTS THAT TO HAPPEN TO HIS OR HER FAMILY.

In most countries before you start work it is an ACT of parliament that you carry out what is called Risk Assessment.

In this Risk Assessment, you determine what could go wrong and take corrective measures to address them and document them. And all workers on the job would be briefed about these before the work starts. Even in emergency situations, this is done before the work is carried out. The work may be abandoned if it is identified that they don't have all the corrective measures in place to carry out the work safely.

For electrical work for example, all isolation points are locked out and every worker on the job will fit his own personal padlocks at that point. After completing the work, your padlock is unlocked by you and removed by you. The law does not allow any other person to remove some other persons padlock. To address this issue the padlocks keys are all different and the padlocks are heavy duty industrial padlocks made from high strength steel.- BULLET PROOF.
In other words the power through the isolated point cannot be turned back on until all the padlocks have been removed. The padlocks are clearly identified and no two keys are the same.

For safety critical plants like VALCO, The Mines, Oil and Gas Industries it is a mandatory requirement that personal protection equipment are provided and inspections are carried out on regular basis. As a requirement stated by the ACT, Serious Safety incidents resulting in amputation, serious personal harm are reported to the Govt.

Ghana should therefore have Health and Safety Inspectors who will ensure that safety issues are identified in our work places and work procedures are documented and trainings carried out on periodic basis.
And also correctives measures are put in place to address safety issues. Personnel Training Records are kept for inspection and review by Govt Officials on periodic basis.
The grant of permit to continue to operate is subject to satisfactory evidence that you have complied with the ACT
The procedures apply to emergency situations well.

Ghanaians in general and the bereaved families should kindly discuss this with their parliamentarians and determine whether the ACT already exists or not. I am sure we have an ACT like this . It may be a question of implementation .
If we don't have something like this, it may be something we should consider
