Causative agent of Guinea worm Disease(Dracunculiasis) is a parasite which causes infection with a parasitic roundworm known as a Guinea worm. The disease manifests a painful and disfiguring wounds. Poliomyelitis (Polio) is caused by the Poliovirus and leads to paralysis.
A little bit of basic research before publication could have saved the embarrassment.

Ancient disease? what is that? is that how medical experts describe diseases? is malaria also an ancient disease? How about cancer? it has also been documented since time immemorial, so wold we call it an ancient disease?.
Is there a village called Denkyira near the capital Accra? We all know about the Denkyira districts and constituency(s) in Central region but I am not sure it can be described as near Accra. Unless there is indeed a village of that name that I am unaware of that is so close to Accra as this report is describing.
I was going to forward this report to my coworkers to see some successful work in Ghana, but after reading this class 1 report It would be a disgrace to Ghana to send this report that even a 5th grader can correct. Twea.
